The Minnesota Safety Council and Governor Tim Pawlenty invite Minnesota employers to participate in the Governor’s Workplace Safety Awards. The awards recognize Minnesota employers for excellence in safety and are presented at a luncheon on the third day of the Minnesota Safety & Health Conference. The application is a secure page and all information you provide is confidential. Incidence rates will be calculated for you automatically (see links to information about rate formulas below).
IMPORTANT: If you are submitting more than one application (one for each of various reporting units within your company) be sure to enter a DISTINCT "Company Name" in each application, e.g. by adding the location or the function of the unit to the company name. (This is especially important if your divisions are at the same address.)
